4th Quarter Fiscal Year 2008

Ingham Health Plan and other health program enrollments:

·                     151 registrants enrolled in the Ingham Health Plan (IHP)

·                     63 had doctor appointments at 2 clinic sites

·                     65 people enrolled in the asthma program

·                     15 pregnant women referred to either Capital Area Community Services, Shared Pregnancy, or Christian Services

·                     100 families enrolled in WIC/Project Fresh

·                     Completed 25 on-line registrations for women, including Michild enrollments

·                     Assisted 300 MSU women with Plan First application process

           

Health Promotions:

·                     African American Parade – 300 health packets distributed

·                     Cristo Rey Picnic – 200 packets distributed

·                     Southside Showcase – 200 packets distributed

·                     Willow Teen Health Fair – 150 packets distributed

·                     Posters hung in 34 south side locations, with 50 fliers left at each of these sites

·                     300 lead screening health packets passed out in door-to-door canvassing

·                     Presentation on chronic illnesses at Southside Community Coalition Senior Luncheon

·                     Conducted health care outreach surveys with 80 families and 30 seniors

·                     150 people received nutrition education food and recipes

·                     Teen Scene participated in Substance Abuse Prevention Program; a grant funded activities and a play that the teens wrote and performed

·                     October Health Fair including Flu shots and screenings for blood pressure, glucose and cholesterol
